Registration / Consultation

    For consultation, patients are required to fill registration form at the Enquiry Desk in OPD Building or Emergency with their full particulars and the name of Consultant, whom they wish to consult. For patients unable to write, assistance is provided for filling the form at the counter.

    Online appointment can be taken for consultants giving prior appointment. Appointment can be sought though our website or our appointment helpline i.e. 70731-11911

    Please enter all details (name, age, phone number and address) as mentioned in the documents to avoid inconvenience, especially in case of medical reimbursement or for issuance of relevant certificates.

    A Unique Hospital Identification Number (UHID) is generated for new patient post submission of registration form and making payment. In case of revisit, it is requested that previous UHID number is mentioned by patient / attendant or their previous medical records are carried.
Admission
  • 1
    If consultant recommends admission of patient, it would be mentioned on the OPD sheet, which needs to be shown at Admission Counter placed in Emergency and IPD Building.
  • 2
    The Hospital has different kinds of bed and room categories that patient/attendant can choose from. However, the allotment is subjected to availability on the date of admission. An advance of three days of accommodation charges shall need to be paid in advance while booking.

    Financial counseling & consent for admission is taken at time of admissions.
  • 4
    On confirmation of bed/room availability, receptionist checks all papers and asks for mode of payment, i.e. cash/credit card/mediclaim/from empanelled organization. Admission is done accordingly.

    If patient is willing to avail Mediclaim facility for admission, the Mediclaim office will verify the approval / denial of the same.
Discharge
  • 1
    The discharge process is initiated only after the patient's consultant deems him/her fit for discharge.
  • 2
    Discharge timing is prior to 12:00 noon.
  • 3
    Before leaving, the patient's attendant needs to check their room/ward bed carefully for any personal belongings, as the hospital is not liable for belongings left behind.
  • 4
    Once discharged, doctor and nurse, dietician will explain the treatment plan that you need to follow.
Payment Guidelines
  • 1
    Hospital Charges are non-negotiable.
  • 2
    Hospital Tariff is available at the OPD/IPD reception, enquiry counter and with Patient Welfare Officer.
  • 3
    Please ensure that all doubts about the charges are clarified before admission at time of financial counseling.
  • 4
    If the patient has to undergo an operation / procedure, the patient / attendant have to deposit the full amount prior to the operation procedure.
  • 5
    Any shortage of initial deposit during emergency admissions has to be cleared within 24 hours of admission or before 12.00 noon of the next day.
  • 6
    Provisional bills will be presented to attendant every 3rd day, the payment should ideally be cleared same day.
General Guidelines

    We request our patient/attendant to keep their mobile phones on silent/vibration mode.
  • 2
    Patients and their attendant are requested to avoid overcrowding in patient area / hospital premises.
  • 3
    Patients are requested to avoid self medication.
  • 4
    The medicines prescribed by doctor would be administered by the nursing staff only.
  • 5
    Restlessness, if any, after medication should be immediately reported to nursing staff on duty.
  • 6
    The instructions given by consultant should be strictly followed.
Waiting Lounges for Attendants

    S.D.M.H. Provides air-conditioned 24 x7 waiting lounges in OPD & IPD buildings for attendants at no cost to them.
  • 2
    One dedicated ICU Attendant Lounge is also created for the attendants of ICU patients. The facility is connect with ICUs for transmission of necessary information.
ATM & Bank
  • 1
    Hospital has 24x7 ATM facilities located beneath the Auditorium facility and outside OPD (R.D.M.C) Buildings.
  • 2
    Union Bank of India is also available inside the campus for any necessary transactions required.
Vishram Grah(Accommodation Facility in Campus)
  • 1
    Air-conditioned in campus accommodation facility is available for patient attendants inside the campus.

    Check-in and Check-out time : 12 noon(only two attendants are permitted in one room)
  • 4
    Attendant may pay at admission counter to avail the room facility. However due to heavy demand and limited rooms, attendant may have to wait till a room is vacated.

    In case of non-availability of VishramGrah rooms, attendants may avail Bunk-Bed facility which is available on 12 hours basis at very nominal rates, inside hospital campus.
Book & Newspaper Shop

    The Book & Newspaper shop is available near the Sarvdharm Temple.
  • 2
    The shop makes available the books and newspapers.
  • 3
    The facility is available Monday to Saturday from 7 am to 9 pm.
Sarvdharm Temple
  • 1
    Sarvdharm Temple is located near waiting area outside IPD.
  • 2
    The temple is open from 6 am to 8 pm every day.
Salon
  • 1
    S.D.M.H. encompasses a salon inside the campus.
  • 2
    The facility is available from 6 am to 10 pm every day.

Health Check up Plan
Visitor’s Policy
SDMH Visitor Policy has been established to provide a supportive and caring atmosphere for the patient. Visitor may visit between the following hours:-

    Visiting Hours 6 am to 8 am and 5 pm to 7 pm
  • 2
    Only one visitor with a visiting pass is allowed at a time.
  • 3
    Children under 12 are not allowed as visitors.
  • 4
    At the time of admission, patient is provided one all time attendant pass and two visiting hours pass.
  • 5
    ICU patients can be visited during the permitted time only for five minutes by two authorized attendents.
  • 6
    Flowers are not allowed inside hospital premises.
  • 7
    On the recommendation of the consultant, an additional pass may be issued on extra payment.
